Be sure to fill out all fields with as much information as possible. Note that these options must be selected to get the appropriate bug report form:

1: "Request type" should be set to Relic Entertainment | Company of Heroes & Dawn of War Series.

2: "Game" must be set to Company of Heroes 3.
